Movie Synopsis
John Milton (Nicolas Cage) is a criminal who has broken out of Hell to kill Jonah King (Billy Burke), a cult leader who tricked Milton's daughter into joining his followers in the wake of Milton's death, only to kill her and her husband and steal their baby daughter - Milton's granddaughter - to be sacrificed in a Satanist ritual.
Milton interrogates some of King's followers, killing two of them brutally while leaving the third one badly injured to spread the word to King that he is coming. He learns from the survivor that they are planning a cultists' ritual at an abandoned prison called Stillwater Marsh in Louisiana, which King plans to use during the full moon in three nights to sacrifice his infant granddaughter. While traveling toward his location, he comes across a diner and meets Piper (Amber Heard), a feisty and fiercely independent woman who had quit her job due to sexual harassment. Needing her car to get to where he needs to be, he causes it to break down, then introduces himself to her for a ride. They get to her home just outside of a truck stop and they part ways, but Piper catches her fiance Frank in bed with another woman, and the fight forces Milton to intervene and beats him up before he can beat Piper up. Out of gratitude, Piper agrees to take Milton to Louisiana. Meanwhile, Frank encounters a strange man in black who calls himself The Accountant (William Fichtner), who has come to take Milton back to hell . He interrogates and kills Frank after finding out what he needs, then tricks two Colorado State Patrol officers into thinking he is a FBI agent and convinces them to shoot-to-kill Milton and anyone traveling with him.
While at a tavern hotel, King's men zero in on Milton, but he turns the tables on them, killing each of them except for one which Piper intervenes before the man can kill him. They inadvertently kill the two officers who try to kill them and flee, effectively turning the Oklahoma state patrol on them as well. The Accountant follows them in the patrol car, threatening to run Milton off the road, but Milton pulls out a large, ancient gun (later called the God Killer) which shocks the Accountant and scars his cheek as he swerves to avoid it, tumbling over the side of the bridge to the ground below. Milton and Piper travel to a church where King's followers and King himself are congregating, and when pleading to give up the baby, Milton is shot in the face and left for dead while Piper is kidnapped and taken into King's RV with the intent of King teaching her about his cult status. But Milton revives from his injury and kills several cultists left behind to bury him before resuming his pursuit. Piper breaks loose in an attempt to get the baby back and gets into a fight with King who proclaims he's going to kill her. She manages to escape by jumping onto her car, but King shoots out the engine block, causing them to stall out. Seeing his wounds and recounting instances where he seemed to have been dead, she begins to suspect he has come back from the grave for his granddaughter. They push the car to a man named Webster (David Morse). Webster can't fix Piper's car, but he gives a newly refurbished vehicle to Milton, he warns Piper that they may die in the mission, but she agrees to stay with him until the end. Enroute to Stillwater Prison, Milton and Piper come across a blockade of trooper cars. With the intervention of the Accountant, however, the blockade is smashed and Milton manages to drive through unimpeded. When they get to Stillwater, the Accountant catches up and forces Milton to give up his God Killer in exchange for the chance of saving the baby and going back to Hell without a fight, it is explained that the God Killer has the power to eradicate anyone's soul from existence, no Heaven, no Hell , nothing.
Using his car, Milton manages to kill nearly all of King's followers, but he is ejected from his car with King and his right hand man still standing. Piper steps in and kills one of them with the God Killer, but the kick of the gun knocks her out and Milton is left to face off against Jonah who beats him mercilessly as he makes his way to the gun. The Accountant distracts King just long enough for Milton to grab the gun and kill him with his last bullet, obliterating his soul and disintegrating most of his body. As he lay dying, Milton says goodbye to his granddaughter and entrusts her with Piper telling them that Webster will take care of them as Webster arrives. He passes away silently while Piper and Webster take the baby, only to turn back and find that he and the Accountant have disappeared.
Unbeknownst to the pair, Milton is still alive, as you can't kill someone who is already dead. He drinks beer from a makeshift cup of part of King's skull, as he said he would, and the two talk about his escaping. Milton tells the Accountant that he plans to escape again someday to which the Accountant is intrigued, and likewise hopes to get in on the chase should that happen. When Milton is ready, the Accountant summons a car, and Milton drives through the portal to Hell .
